What is the Procedure for Carpal tunnel release surgery?

If the symptoms due to carpal tunnel syndrome are very long-lasting and getting worse, carpal tunnel release surgery is a good idea to prevent further deterioration and still retain the normal functioning of the hand. The doctor will inform you about the procedure, complications, and prognosis and decide on a date for the surgery. A day before the surgery, one may be admitted, the necessary tests are done, and consents are signed. You may be asked to stop eating at midnight, and the next day, after shifting to the operation theatre, the operation will be done. During the operation, the following things will happen:

  1. Before the procedure, you are changed into the hospital gown, and scrubbing and cleaning of the area to be operated on are done. The doctor will ask about any significant medical history or allergies to anaesthesia.
  2. The surgeon then induces anaesthesia, and after cleaning and draping the part, cuts the skin over the wrist to dissect and expose the flexor retinaculum.
  3. The band is then slowly cut to release the pressure over the nerve, and any other structures compressing the nerve will also be removed.
  4. Once the procedure is done and there is no extra bleeding or complications, the incision is closed up layer by layer, ending with skin sutures, and a bandage is applied over it.
  5. Once the operation is over, you may be kept in observation for 2–3 hours and then sent back to the ward. Depending on your recovery, you may be discharged the next day or the day after that.

What are the complications of CTS surgery?

Just like any surgery, there are some complications that may happen in a carpal tunnel release surgery:

  1. Excessive bleeding
  2. Damage to surrounding structures
  3. Damage to the median nerve
  4. Infections
  5. Allergy to anaesthesia
  6. Very rarely, complex regional pain syndrome occurs where the condition doesn’t go away and may spread to the rest of the arm and forearm along with skin changes, severe pain and numbness.

Carpal Tunnel Syndrome Symptoms:

The symptoms experienced in carpal tunnel syndrome mainly affect the lateral three fingers and the surface of the palm, covering the nerve distribution of the median nerve over the hand.

  1. Pain is low to moderate in intensity over the hand affected.
  2. A tingling sensation over the palm and the sensation of electrical shocks
  3. Loss of grip due to weakness of the muscles of the hand
  4. There may be an increase in symptoms and pain during the night.
  5. A decreased perception of cold and hot items may be present.
  6. If not treated on time, the pain can spread to the forearm and arm as well.

These symptoms could be carpal tunnel syndrome or even some other disease; therefore, it is extremely important to visit an orthopaedic surgeon for the right diagnosis and further management.

Carpal Tunnel Syndrome Treatment:

Treatment for carpal tunnel syndrome can be both medical and surgical in nature.

  1. Changes in lifestyle: taking brakes during activities that intensify the condition, doing streching of the wrists, maintaining good posture
  2. physiotherapy to restore strength and range of movement.
  3. a corticostreroid injection in the wrist to soothe the inflammation in the joints.
  4. Use of pain medication, such as non-steroidal anti-inflammatory drugs, to manage the pain during everyday activity
  5. wearing wrist splints to immobilise the wrist, prevent it from getting reinjured, or allow it time to heal.
  6. Surgery may also be done in cases where the condition is not being managed with the help of conservative methods. The surgery performed is called a carpal tunnel release surgery, where the flexor retinaculum, or the band of fibres running over the median nerve, is cut to release pressure on the nerve, allowing it to heal.

Diagnosis of carpal tunnel syndrome:

On a visit to the orthopaedic surgeon,he will ask you about the onset, duration, intensity of symptoms, your occupation, area of pain, and factors aggravating and relieving the pain or symptoms.
The orthopaedic surgeon may also do a clinical examination to further evaluate the patient, which can include tests like the phalen’s manuveurband tinels test. Phalen’s manoeuvre involves flexing the wrists and holding them in a bent position for a minute.
If the pain increases or symptoms arise, then one may have CTS. Tinel’s test involves pressing on the median nerve at the wrist, and that reproduces the symptoms, confirming CTS. For further confirmation, the doctor might also advise the following investigations:

  1. Nerve conduction studies are done to study the amount of nerve damage and the conduction of electrical impulses through the nerve.
  2. Electromyography is a study done to check for any muscular damage or weakness that may be because of a condition other than CTS.
  3. For a more detailed view of the wrist, especially before surgery, the doctor may advise an MRI of the wrist.
  4. The doctor can also write tests like hormonal essays, blood iron, a complete blood picture, and so on.
  5. If there is a plan for surgery, then the doctor may advise a chest x-ray, ecg, and 2D echo as well to eliminate the possibility of cardiovascular abnormalities.

How do I recover after a CTS surgery?

Recovery after carpal tunnel release surgery requires a properly guided recovery plan with diet changes and proper post-operative care.

  1. Eat a nutritious, protein-rich, and fibre-rich diet.
  2. Avoid strenuous activity and lifting heavy weights for at least a month with the operated wrist.
  3. Get regular physiotherapy to return strength, flexibility, and mobility to the hand.
  4. Take medication on time.
  5. Wear compressive bandages or splintsas prescribed to control swelling and aid recovery.
  6. Come for regular follow-ups.
  7. Stay hydrated and get plenty of rest with regular sleep every night.
  8. If you notice any fever, pain, or oozing from the wound, alert your doctor.

FAQs

Although carpal tunnel syndrome can affect people of any age, occupation, or gender, it is seen in higher numbers in people with the following:

  1. People in occupations like data entry, typing, and heavy machinery
  2. People with thyroid issues, rheumatoid arthritis, obesity, and autoimmune conditions
  3. Pregnancy causes fluid retention and swelling in the body and can therefore cause carpal tunnel syndrome.
  4. People with a history of carpal tunnel syndrome in their family
  5. It is also more often seen in older adults and in females.

The carpal tunnel surgery procedure time is about 40–45 minutes in an uncomplicated case and may take up to 1 or 1 and a half hours at the longest.

For a person who has already been treated for carpal tunnel syndrome, it may come back if the risk factors for the condition are not eliminated. Chronic cases of carpal tunnel syndrome require carpal tunnel release surgery.

The cost of carpal tunnel surgery in India depends on the doctor, the complexity of the procedure, the area, and the hospital where surgery is done.
