What is BPH(Benign prostatic hyperplasia)?

Benign prostatic hyperplasia is the excessive growth of the prostate gland during old age. The prostate gland surrounds the male urinary tract, and therefore excessive growth of prostate tissue can lead to compression of the tract, leading to urinary problems. It is mostly an issue for above 60 years old people. There are a variety of surgeries available that are done on a day-care basis, that is, the person can leave the same day that they are admitted or in 2–3 days, depending on the procedure and other conditions of the patient.

What are the different modern BPH Surgeries?

There are several minimally invasive procedures for an enlarged prostate, including TURP, TUIP, and HoLEP. TURP and TUIP are minimally invasive surgical procedures, and HoLeo is a laser-based procedure.

1. Transurethral Incision of the Prostate (TUIP):
It is a surgical method performed under anaesthesia in which the surgeon inserts an instrument called a rectoscope into the urine pipe or urethra to visualise the anatomy of the pipe. Based on the visuals from the rectoscope, the surgeon will make an incision to widen the canal through which the urine passes. It is a minimally invasive surgical intervention that is commonly used in younger males who wish to preserve their fertility. The doctor may place a urine catheter to drain the urine while the tract heals. The relief due to the procedure is almost immediate, and the benefits are long-lasting.

2. Transurethral Resection of the Prostate (TURP):
TURP is also a popular method to rectify urinary symptoms due to an enlarged prostate. In this process, the surgeon, under local anesthesia, inserts a resectoscope into the urethra through the penis. Once inserted, the surgeon tried to burn off the excess tissue by inserting a hot wire through which an electric current passes. This is called cautery. By removing the excess tissue, the urinary canal becomes wide, and urine passes easily. A catheter is also placed, which is then removed later.

3. Holmium Laser Enucleation of the Prostate (HoLEP):
HoLEP is a laser-based procedure to treat an enlarged prostate. After administering anesthesia, the surgeon inserts a resectoscope, and instead of making an incision or burning the tissue, an advanced holmium laser is used to destroy the excess prostate tissue. The use of a laser does not require cutting. treat urinary symptoms caused by an enlarged prostate. In this procedure, the urologist inserts a thin, tube-like instrument called a resectoscope. The surgeon uses an advanced Holmium laser to destroy the excess prostate tissue without the need for any incisions. The patient is given anesthesia before the beginning of the procedure. The surgeon may choose to place a catheter, which may be removed later.

FAQs

BPH in its initial stages can be managed with medications like alpha blockers and DHT blockers, but as the condition advances, surgery is necessary for long-term resolution.

The pain at the surgery site lasts for 2–3 days. A slight burning sensation may also be experienced during the first few days after surgery. The pain is usually managed with mild painkillers before and after the surgery.

You can return to your regular daily activities within a week following surgery. For more strenuous work, more time for recovery may be required.

TURP may cost anywhere from 70, 000 to 1,00,000 rupees depending on the city, procedure, cost, etc.
