  • Formation and drainage of pus on the skin

How is an anal Fistula Diagnosed?

  • The diagnosis of an anal fistula can be made via the following techniques:

    1. A clinical examination by the doctor
    2. MRI, or magnetic resonance imaging, can give detailed insight into the tract of the fistula and its relation to surrounding structures.
    3. Endorectal ultrasound can help identify the location of the fistula.
    4. Fistulography is done using X-rays with contrast to identify the tunnel.
    5. Anoscope to see the internal opening of the sphincter
    6. Fistula probe: a metal instrument used to identify the fistula tunnel.

What are the causes of Anal fistula?

  • Sexually transmitted infections(STIs)– In most cases, sexually transmitted diseases increase the risk of anal fistula in an individual. Sexually circulated infections may include Chlamydia and syphilis that contributes to the formation of puss in the anus.
  • Tuberculosis- Perianal tuberculosis is a very rare condition. When TB spreads beyond the limit, it may affect the area around your anus. Thus causing trouble in the perianal region.
  • Perianal abscess- Major cause of Anal fistula is perianal abscess. An abscess is nothing but a sack of puss that forms and infects the anal region. Even though the abscess is removed still some fistulas are left unidentified which leads to the creation of new fistulas and anal cancer.
  • Crohn’s disease- Categorised under Inflammatory bowel disease(IBD). This causes inflammation(swelling) in the tissues of the digestive tract system when the body’s immune system attacks healthy tissues falsely. Foods containing high fibre and lactose can trigger this.
  • Hidradenitis suppurativa- HS is a prolonged condition related to the skin that leads to the formation of abscess/puss on the skin. This is caused by to blockage of follicles. Smoking and obesity are the main factors.
  • Past injury and Radiation therapy- If you ever had a traumatic or severe injury in your anus then the risk of fistula increases to an extent. Radiation therapy is recommended to treat anal cancer in your pelvic region. Due to this inflammation is caused in the anus and rectum. It damages the tissues present in it.

What are the treatments for Anal fistula?

  • Perianal fistula surgery- Also known as Fistulotomy. This surgeon cuts a whole long fistula and keeps it open so that it can heal for a certain period and turn into a scar. Stitches are done very rarely only to narrow the cuts or wounds instead of completely closing them.
  •  Laser treatment for fistula- Fistula laser surgery involves the usage of laser energy emitting through a radial fibre that focuses on the fistulas and forces the damaged tissues to shrink thus contributing to the healing process.
  • Fistulectomy- Unlike fistulotomy, in this, the whole fistula tract is removed or detached completely to eliminate abscess and damage to the anus. Because this damages the sphincters (circular muscles) it is not often recommended but does create damage every time.
  • LIFT Surgery- Refers to Ligation of the intersphincteric tract as a treatment, especially for anal sphincter muscles. Sphincter muscles are circular muscles that open and close the opening to prevent the backward flow of intraluminal contents. When fistulas develop in the sphincter muscles it may not allow them to function properly. For this incisions are made above the skin and sphincter muscles are separated.
  • Seton techniques- If the fistulas affect specific parts of sphincter muscles then seton techniques are advised. Seton is a surgical instrument in the form of a thread that is left in the fistula to make sure it is opened for some weeks. This helps in healing as no cuts are made.

FAQs

The main cause of fistula is infection and formation of abscess/puss around the anus.

The chances of recurrence are meagre, but a new fistula may develop at a different site, or the same fistula may reopen. A fistulotomy is done to deal with the already-occurring fistula, not as a preventive surgery for the formation of further fistulae.

In about one week post-surgery, you should be able to do your normal daily activities while continuing to take precautions such as keeping the wound clean and dry, avoiding straining during defecation, eating a fibre-rich diet, and also informing the doctor if you see an increase in discharge, blood or pus at the site of surgery, or a fever.

To determine the size and location of the fistula, anorectal surgeons may perform an MRI or ultrasound of the Anus, Anoscopy and Colonoscopy.

Fistula is more common in males than females. Nearly men at the age of 40 are at the risk of anal fistula.

Rectal fistula happens due to the infection around the anus that creates puss and opens a small tunnel near the perianal region. Anorectal fistula surgery is performed to treat this.

The only alternative to surgery is the usage of Fibrin glue. Surgeons inject glue into the fistula to seal it which helps in healing.

The cost of anal fistula surgery is anywhere between 30,000 and 50,000 rupees in Hyderabad. It can vary based on the type of procedure, the experience of the doctor, and the complications associated with the fistula.

Fistula is an abnormal opening in the anus whereas a fissure is a small cut in the inner lining of the anus.

Avoid any sexual activity, Intense exercises, and tight clothing, you need to apply prescribed ointments and sit in warm water for 15 minutes(sitz bath).
