Any kind of forcible pressure that twists or bends your knee exceeding its natural capability may lead to ACL tear or injury. What are the Types of ACL injuries

The type of ACL tear or injury helps in determining the severity of the tear and suitable treatment options. The different grades of ACL tear are:

  • Grade 1 ACL Tear: mild, ligament stretched but not torn, minimal instability. It can be treated using the RICE model- rest, ice, compression and elevation.
  • Grade 2 ACL Tear: Partial tear of the anterior cruciate ligament, some fibres torn, moderate instability. A combination of non-surgical treatments such as physical therapy, bracing, activity modification and the RICE model are used to treat grade-2 ACL tear.
  • Grade 3 ACL Tear: complete tear, significant instability, often requires surgery.

What are the Complications of ACL tear surgery

Only 2%-10% people experience the complications after an ACL tear surgery where infection is most common.

  • Risk of infection and blood clots
  • Graft failure causing joint instability
  • Reduced motion of leg due to stiffness
  • Chronic pain, numbing or tingling sensation in the knee
  • Nerve or vascular damage

What are the Procedure & Diagnosis of ACL Tear and Surgery

The type of ACL tear surgery will be recommended based on the type of ACL tear and the result of the diagnosis.

Diagnosis: The diagnosis starts with a physical examination of your knee (where your doctor might ask you to perform certain movements), followed by quick questions regarding the symptoms and injury. A Few tests, like an X-ray, CT scan, and MRI may also be recommended for confirming the diagnosis and assessing the severity of ACL tear.

Procedure: The surgical treatment is done for grade-3 Acl tear after considering the patient’s medical history, age, the surgeon’s experience and expertise, etc. The procedure takes around 2-3 hours and is performed under local, general, or regional anaesthesia. There are mainly two types of anterior cruciate ligament reconstruction surgery :

  • ACL reconstruction: it involves creating a new ACL using a graft, typically from the patient’s tissue (autograft) or a donor (allograft). The common graft sources include the patellar tendon, hamstring tendon, or quadriceps tendon for autografts and cadaver tendons for allografts. It is also known as ACL rupture surgery.
  • ACL repair: it involves suturing or reattaching the torn ACL to promote healing and stability. This may be suitable for certain types of partial tears or specific tear locations depending on the quality of the ACL tissue.
