What is an ACL Tear?

The ACL is the anterior cruciate ligament, which maintains the stability of the knee joint along with other ligaments in the knee and prevents the knee from sliding forward during everyday activity. Sometimes, due to severe twisting of the knee or forceful anterior dislocation of the knee, as in an accident, the anterior cruciate ligament can tear. This reduces knee stability and can cause a giving-away sensation. The ACL tear needs to be repaired as soon as possible because, if left untreated,it may lead to early-onset arthiritis and tears of other ligaments, like the medial meniscus.
ACL repair or ACL reconstruction from other graft materials is a very skilled and delicate surgery that therefore needs to be done with a lot of care and precision. Make it possible by getting an appointment with our team of the best ACL surgeons in India. Book yours today!

Best Doctors For Acl Surgery

How is an ACL tear diagnosed?

For the diagnosis of an ACL tear, the orthopaedic surgeon will first take the patient’s history and ask about any known injuries, the intensity of pain, instability in the knee, locking of the knee, sharp pain in the knee, and other joint-related issues. The orthopaedic surgeon will also do some clinical examinations to detect signs of a ruptured ACL, such as a positive anterior drawer test and a positive Lachmann test. If elicited,he might send for further investigation to confirm the diagnosis.
Some of the investigations ordered are as follows:
1. X-ray the knee to check for any dislocations.
2. An MRI of the knee to check for the state of the ligaments in the knee joint
3. A USG knee is done to visualise the sinovial and externally blocked ligaments.

The doctor may also advise further investigation to check for cardiac fitness for surgery, such as an x-ray of the chest,an ECG, and a 2D Echo. Once a correct idea about the knee is present, the surgery is planned accordingly.

What is the procedure for ACL repair surgery?

After the diagnosis of an ACL tear is made, the orthopaedic surgeon will explain the condition and its implications to you and discuss the best possible surgery and programme for the ACL repair. Once the most appropriate procedure for the injury is decided, the patient is admitted a day before the surgery, and all the necessary investigations are done. Any anxiety or concern related to the procedure will be addressed by the doctor, and printed consents may be taken for the procedure after explaining all the risks and complications that may arise as a result of the surgery. On the night or evening prior to the surgery, an anaesthetist does a complete anaesthetic fitness evaluation to see if the patient can tolerate anaesthesia during surgery. After the surgery is completed, the patient may be kept for observation in an ICU. If the vitals of the patient post-surgery are stable and no complications are seen, the patient can be shifted to the ward, and physical rehabilitation after the surgery can be started. After 2–3 days of hospitalisation, the patient can be discharged with discharge advice.

What are the complications of ACL repair surgery?

Like any surgery, there can also be complications related to the ACL repair surgery. Some of them are as follows:
1. Infection of the joint or the wound
2. excessive bleeding within the joint or hematoma formation.
3. Reduced range of motion or stiffness
4. Damage to vessels and surrounding nerves
5. Failure of the graft or re-injury to the graft due to excessive stress on the knee or improper rehabilitation
To prevent complications from occurring, proper rehabilitation, regular follow-up with the doctors, and good postoperative care are required.

What are the grades of ACL tears?

There are three grades of ACL based on the breadth or thickness of the ACL involved in the tear.
1. An ACL sprain or grade one tear wherein the ACL ligaments get overstretched It can be managed with a conservative approach.
2. A partial ACL tear, or grade 2 tear, wherein the ACL is partially torn through its thickness. This can also be managed with a conservative approach and physiotherapy.
3. A complete ACL tear, or grade 3 tear, in which the entire ACL gets torn and the ligament floats in the synovial fluid. A complete ACL tear needs surgical intervention

What are the types of ACL repair surgeries?

Based on the technique applied during the surgery and the type of repair,ACL repair surgeries can be of different types, such as:
1. ACL repair surgery, in which a partially torn or relatively healthy ACL is stitched back together to facilitate healing. It is not very commonly performed.
2. ACL reconstruction: This surgery is done to reconstruct a new ACL from a different material called the graft, which could be a muscle, tendon, or even someone else’s tissues. It involves taking the graft and placing it in the position of the normal ACL by drilling holes into the tibia and femur and securing it with screws or other fixtures. It is a very common procedure for a complete ACL tear.
3. BEAR, or bridge-enhanced ACl repair, in which an artificially created sponge-like substance is placed between the torn ends of the ligaments and infused with blood. The blood has healing factors that help speed up healing and repair in the ligament. It is still an experimental technique.

The approach for surgical repair of the ACL depends on the level of activity of the patient, the skills and practise of the doctor, the severity of the tear, and other factors.

How do I recover well after ACL surgery?

The recovery after meniscus surgery takes about 2–4 months. The recovery is slow and tedious. However, the following steps can be taken for a better recovery:
1. Regular physiotherapy from a physiotherapist trained in ACL surgery rehabilitation to restore mobility, strength, and balance in the operated knee.
2. A nutritious diet rich in protein to aid the patient’s proper recovery
3. Ample rest, elevation of the operated limb, ice, and compression to reduce inflammation
4. Managing pain effectively with the help of prescribed pain medication
5. Avoid high-impact activities and pivoting movements to prevent reinjury to the graft.

FAQs

The success rate of ACL repair and reconstruction is between 85 and 95%, depending on the type of repair, the strength of the graft, physiotherapy, the patient’s physical activity, and the strength of the fixtures in the bones.

ACL surgery is done under anaesthesia; therefore, there is no pain during surgery, but after the surgery, pain relief is an important component of postoperative care and management. The doctor may prescribe over-the-counter painkillers to alleviate the pain.

Most procedures may take 2–3 hours, but the duration also depends on the type of graft you are taking to replace the ACL.

An ACL reconstruction surgery can cost anywhere from 2.5 to 4 lakhs in India, depending on the type of surgery, the graft used, and the surgeon performing the surgery.
