What are indirect Coombs tests?
The indirect Coombs test, commonly known as the indirect antiglobulin test (IAT), is a laboratory test. It is used to determine whether or not there are antibodies in the blood that are directed against antigens located on red blood cells. This procedure aids in determining blood transfusion compatibility, Rh incompatibility between a pregnant woman and her child, and the possibility of hemolytic anemia caused by an autoimmune disorder. The indirect Coombs test works on the antigen-antibody response between incompatible blood types. The blood samples are treated with a variety of collected or manufactured human RBC antigens before being examined under a microscope. If RBCs agglutinate (clump) after being exposed to a specific antigen, antibodies against that antigen are already present in the blood samples. Antibodies to RBC antigens A and B exist naturally; however, antibodies to additional RBC antigens are formed upon antigen exposure. The Direct Coombs Test detects the existence of antibodies (bound to RBCs) against the body’s own RBCs (autoimmune antibodies), which may be the cause of autoimmune hemolytic anemia.
