What is a Coombs test?

Coombs test is done in pregnancy for Rh isoimmunization(hyperlink). It is a disorder in which, due to some reason in the current pregnancy or past pregnancy, there was the exposure of mother smothers or maternal blood, which is Rh-negative to fetal that is Rh-positive. The formation of antibodies in the mother smothers blood against fetal RBCs. These antibodies stay lifelong and can cross into the fetus from the mother via the placenta. On reaching the fetus, they attack the fetus’s red blood cells and cause their destruction. These antibodies can cause anemia in the fetus, a cause of jaundice(hyperlink) in a newborn fetus, and a dreadful condition called hydrops fetalis, which is fatal. Indirect Coombs test in the mother can help detect the presence of these antibodies in the mother so that they can be neutralized. If Rh-isoimmunization is identified, the fetus in the womb is tested for anemia; we can start treating it in the womb itself before disastrous consequences.

How is a Coombs test done?

The Coombs is a simple test. A phlebotomist withdraws blood from one of the veins and takes it to the Lab. Antibodies against RBCs are tested in the Lab by putting the blood plasma on a slide containing others RBCs. After this, antibody to human antibodies is added, which bind to the antibodies that may be present in the test sample. The fall is then examined under the microscope, and if the cells have clumped or agglutinated to get, the Indirect Coombs test is taken as positive.
