How is the diagnosis of chronic sinus and nasal conditions made?
One should visit the ENT specialist if any of these problems are present:
- recurrent URTI
- Mass in the nasal cavity
- Office headaches or morning headaches
- Inability to breathe properly through the nose
The doctor will then ask about the duration, intensity, and progression of the disease. They may also ask about any associated illnesses and do a clinical examination to visualise the nose, oral cavity, and throat. Based on the findings and a provisional diagnosis, the doctor may ask you to complete some investigations, such as:
- X-ray of the sinuses
- CT head
- Blood tests
- Allergy testS
- Endoscopy
- X-ray of the chest
- ECG
Once all the investigations are done, the doctor may recommend medical treatment or a plan for a surgery such as FESS combined with another procedure such as turbinectomy, osteotomy, polypectomy, and so on, depending on the problem of the case.
What is the procedure for FESS?
The procedure for FESS is as follows:
- The patient is placed under anesthesia, and the vitals are monitored.
- The part to be operated on is draped and painted.
- The patient is positioned on the operating table with the head slightly tilted backwards, giving a better view of the nasal cavity and nasopharynx.
- The endoscope is put inside the nose through one of the nostrils, and the sinuses and their openings are visualized.
- The surgeon then removes any obstacles or obstructions in the nasal cavity causing the breathing issues, such as a polyp.
- The surgeon may then enlarge the natural openings of the sinus cavities or make new openings for better ventilation and drainage of the sinuses.
- They may also correct any septal deviations if necessary.
- Once the main part of the surgery is done, the sinuses are cleaned, the nasal cavity is cleaned, and closing sutures are placed.
- Nasal packing and bandaging are then done to provide compression and protection to the surgical wounds.
The patient is shifted to an observation room, where doctors monitor the patient for 2 to 3 hours to look for any complications. If the patient is recovering well from anesthesia with no immediate postoperative complications, they are shifted to their ward room.

What are the complications of FESS surgery?
The following complications can be seen in a FESS operation:
- excessive bleeding during the operation or 24 hours after the operation
- Infections of the surgical site

- Damage to nearby nasal structures such as the nasal septum and air cells
- Damage to the cribriform plate and CSF leakage
- Damage to the optic nerves in a rare scenario
One should discuss all the possible complications and side effects of the operation before opting for a FESS procedure.
When does an ENT surgeon suggest FESS?
FESS is a precise and advanced method to clear out any blockages in the sinuses and obstructions in the nasal cavity; therefore, for any condition where there might be an obstruction in the nasal part of the airway, FESS is a suggested surgery. Some of those conditions are listed as follows:
- Nasal polyp
- Mastoid sinusitis
- Nasal septum deviation
- Hypertrophied nasal turbinates
- Nasal bone fracture
- Cribriform plate fracture
- FESS-guided artery cauterization for excessive nose bleeding
- Sinus Mucocele
